About Your Work

We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our medical clinic team. As an LCSW, you will play a vital role in providing holistic and patient-centered care by addressing the psychological, emotional, and social aspects of our patients' health. You will collaborate with medical professionals to enhance patient outcomes and contribute to a collaborative and supportive healthcare environment.

You will focus on:

  • Psychosocial Assessments: Conduct thorough psychosocial assessments of patients to identify emotional, social, and mental health factors that may impact their overall well-being and medical treatment.
  • Crisis Intervention: Provide immediate crisis intervention and support to patients facing urgent emotional or psychological distress, collaborating with the medical team to ensure appropriate care.
  • Individual and Group Therapy: Offer evidence-based therapeutic interventions to patients through individual and group therapy sessions, addressing issues such as anxiety, depression, grief, adjustment, and coping with medical conditions.
  • Treatment Planning: Collaborate with patients, families, and the medical team to develop comprehensive treatment plans that integrate medical and psychosocial care, focusing on patients' strengths and needs.
  • Behavioral Health Support: Assist patients in managing chronic conditions by providing guidance on behavior modification, stress reduction, and coping strategies to improve overall health outcomes.
  • Patient Education: Educate patients and their families about mental health resources, coping skills, community support, and psychoeducation related to medical conditions.
  • Documentation: Maintain accurate and up-to-date clinical documentation in compliance with legal and ethical standards, ensuring confidentiality and information security.
  • Interdisciplinary Collaboration: Collaborate with physicians, nurses, social workers, case managers, and other healthcare professionals to facilitate a coordinated and integrated approach to patient care.
  • Referrals and Resource Navigation: Make appropriate referrals to community resources, mental health agencies, and other support services based on patients' needs.
  • Ethical and Legal Standards: Adhere to ethical guidelines and legal requirements in the field of clinical social work, maintaining professionalism and integrity in all interactions. The Skills, Experience & Education You Bring
    • Master's degree in Social Work (MSW) from an accredited institution.
    • LCSW licensure in the state of California is required.
    • 1- 2 years of experience as an LCSW, preferably in a medical or healthcare setting.
    • Experience in crisis intervention, medical social work, and chronic illness management is a plus.
    • Additional certifications in specialties such as trauma-informed care, grief counseling, or medical social work are desirable.
    • Clinical Skills: Strong clinical assessment and diagnostic skills, with expertise in evidence-based therapeutic modalities for individuals and groups.
    • Communication: Excellent verbal and written communication skills, with the ability to communicate complex emotional and psychological information effectively to patients, families, and medical staff.
    • Empathy: Demonstrated empathy, cultural sensitivity, and the ability to establish rapport with diverse patient populations.
    • Team Player: Ability to work collaboratively within an interdisciplinary healthcare team and contribute positively to a supportive work environment.

Final determination of a successful candidate's starting pay will vary based on several factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.         Physical Requirements: Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork - Activity level: Sedentary, frequency most of workday.
